Let’s Build Your End-of-Life (EOL) Plan.

All new clients of EOL Blueprint Advisors engage in an intake session. Each intake session is designed to help you clarify your intentions in working with EOL Blueprint Advisors, identify the aspects of your end-of-life journey that are worrying or concerning you, and clarify the services that you need to help build an end-of-life blueprint unique to you.

All EOL Blueprint Intake Sessions include a welcome discussion lasting up to 90 minutes and the administration of a short pre-survey to help clarify some of your present-day concerns about death and dying.

EOL Blueprint Advisors will assist you in (a) evaluating whether your documentation is in place to support your end-of-life goals, (b) completing recommended documents, and (c) communicating your documents with your care team. This includes:

1. Reviewing and/or assisting you with completing healthcare proxy designation.

2. Reviewing and/or assisting you with completing your goals of care document.

3. Reviewing and/or assisting you with completing durable/financial power of attorney designation(s).

4. Reviewing and/or referring you to professional to complete life insurance, will/estate planning, and recommended financial documents.

Clients without a healthcare proxy and/or goals of care, will be guided through completing their Five Wishes documentation. Pricing includes the cost of downloading and storing Five Wishes documentation.

Optional add-ons include:

1. Planning, co-facilitating, and documenting a conversation with loved ones about your Five Wishes

EOL Blueprint Advisors will help you set up and manage a secure document locker for storing all the material you and your loved ones will need to manage your personal and administrative affairs through end-of-life. Pricing does not include the $8 monthly storage fee for the preferred EOL Blueprint Advisors secure document platform.

EOL Blueprint Advisors will help you develop and execute a plan for communicating your end-of-life plan with the loved ones who will become part of your end-of-life support team. Services will include:

1. Developing the roles, goals, and expectations of your end-of-life support team.

2. Developing a data-driven plan for communicating your end-of-life goal with your support team.

3. Planning, co-facilitating, and documenting meetings with you and the loved ones on your end-of-life support team.

4. Assisting with setting up a CaringBridge or like site for coordinating group collaboration and sharing during your end-of-life journey.

IteEOL Blueprint Advisors will engage you in life review and narrative therapy to help you articulate the legacy you have created in your life. This process will include: (a) articulating highlights and points of pride in your life to date, and (b) identifying areas where you might have unfinished business to attend to, including people with whom you need closure in your life. Every EOL Legacy participant will receive a document that summarizes their legacy highlights, and offers an action plan for addressing unfinished business.

Optional Add-ons include:

1. Coaching to help you prepare to share your EOL legacy with your loved ones

2. Development of your obituary to have ready following your eventual death

3. Development of a personalized legacy document(s)/project to share with your loved ones

4. Assistance preparing for important conversations to bring closure to key relationships m description

EOL Blueprint Advisors can help you review options for disposing of your mortal remains, and help you develop and document a funeral and burial plan consistent with your values, legacy, and financial means. This service includes helping you (a) select a funeral provider and related details, (b) document your wishes for a memorial service or celebration of life, and (c) communicate your funeral plans with your loved ones.
